Class field-trips are an important part of our curriculum. They strengthen connections and a sense of community in the classrooms; among the kids and the adults.
We are continually working on improving the extensive process that goes into these field-trips. Here are some of the need to know requirements that are consistent for each and every field-trip.
Permission Slips are to be signed an returned to the classroom teacher directly.
Payments should be made at the front desk or on the website.
Cash, Checks or Cards are accepted. Checks should be made out to BLUE OAK SCHOOL. A receipt will be made reflecting the Class teacher’s name, field-trip and student name. To make a payment on the website click HERE.
Make sure to read the directions before clicking the payment button to ensure the teacher name is entered.
ALL drivers need to have a fingerprint clearance and all required driver info. on file with the front desk. Information can be dropped off at the front desk, emailed to email@example.com or faxed to 530.879.7490. For more details on the requirements or how to complete a Live Scan click HERE.
Field Trip Medication Forms are handed out with the Permission Slips. Please read them carefully. They need to be completed and returned to the Health Aide, or the front desk, one week before the trip date. If you do not already have an “Authorization to Administer Medication” form on file with the Health office, you will need to pick one up at the front desk ASAP, have it signed by a physician, and attach it to this field-trip form.
